Midwest Honor Flight — Crew Application

NOTICE: Effective immediately and as voted by the Board of Directors in the spring of 2025, all volunteers wishing to be part of the Medical Team or as a Bus Captain who have not previously flown must fly as a Guardian first to get a better understanding of the day on flight. This change has been implemented by our insurance and our board to better prepare our Flight Crew Teams for what to expect on flight and to continue to ensure the safety of our Veterans on each Mission.

​Because they play such a significant role in our program, Midwest Honor Flight reserves the right to disqualify any Crew Member based upon the requirements listed above or require additional information/medical screening to ensure the safety of all Veterans on flight.

You will need:

  • FLIGHT CREW & MEDICAL TEAM must attend one mandatory in-person training prior to flight
  • Crew cannot be the spouse or significant other of any Veteran on flight
  • Crew must be between the age of 18-75 at the time of the flight
  • Crew must be physically capable of performing their assigned role, which includes assisting any and all Veterans getting in/out of seats and honorchairs (wheelchair); up/down escalators; and up/down stairs
  • Medics specifically must be qualified to perform the duties of a Medic and have medical experience
  • Crew must be able to push a Veteran in an honorchair for a minimum of one hour at a time including up and down inclines (estimated at 8 miles throughout the entire day)
  • Crew must be capable of independent communication with Veterans and Midwest Honor Flight
  • Crew must not require use of canes, crutches, walkers, slings, personal oxygen concentrators, have back issues, casts, or other limiting physical strength and agility
  • Valid government-issued photo ID

Failure to completely fill out the entirety of this application may result in my application being return to me or not considered for flights
I also understand that I must bring a current form of government issued photo I.D. on flight day, that must not be expired, and will be REAL ID compliant
I understand that before I am confirmed as a part of the Flight Crew, I must make a minimum donation to Midwest Honor Flight with the donation amount and deadline to be announced at the time of selection
